FISA courts/judges aren't investigators. They and the FISA application rules rely on the true investigators to truthfully, under penalty of perjury, present and certify/verify to the court in their applications the facts found in their investigations. The court either grants or not the application.
The court requires written minimization procedures before approving a FISA warrant

Looks like this was standard operating procedure in the FBI/DOJ. I seem to recall Hildebitch's exoneration was written 2 months before she was interviewed.

The curious thing is that none of the FISC judges seem to be concerned with being deceived numerous times. It's possible they have been and expressed themselves behind the scenes, however, very little stays secret in DC. Being reprimanded or sanctioned for committing a fraud on the court by any of the usual suspects likely would have been divulged or leaked by now.
