Difference between wrong and fake reporting

Question:

We all rail about 'fake news'. You hear it from the left and the right. What is the difference between incorrect reporting and fake news? The NYTimes ran with a story two weeks ago backed by its anonymous sources about Rosenstein about to be fired by Trump. You had senators trying to pass legislation to protect his work. You had media hysteria about it (it even made the national nightly newscasts). Two weeks later, Rosenstein still hasn't been fired, and in fact, quotes from Trump are that "they have a great relationship."

My question is how is this not 'fake news' from the NYTimes? How is this any different from the reporting done by Alex Jones?
 
Do you know that their reporting is wrong or fake? Do trump and Rosenstein have a great relationship?

One could easily doubt that
 
Any article that quotes anonymous sources and doesn't attribute to a specific person or have real external corroboration should have to have the 'Rumor' tag on the headline because that's exactly what it happening here. Just gossip mongering.

I 100% believe NYT had an anonymous source that told them this so it is not necessarily “fake news”. NYT, CNN, etc are active members of the DNC and hope everyday to sink Trump. When they hear a rumor from a “source” that they want to be true, they run with it.

In most of these cases it is not fake news, rather just terrible journalism.

Well, they were obviously wrong. Report was he was in his way to be fired that morning. It did not happen, so at the very least they were wrong.

I'd provide links, but NYTimes is behind a paywall. That said, I don't think I've misrepresented any of the statements from the NYTimes that stated he was either resigning or being fired. This was big news on Sept. 24th. Do your own google search to see what I'm referring to if you (like Dr. Ford) aren't capable of remembering things from more than 2 or 3 days ago.
